Who Can Submit a Referral?

We accept referrals from:

  • Therapists and counselors
  • Case managers
  • Social workers
  • Hospitals and clinics
  • Community partners
  • Family members and loved ones

What Happens After You Submit a Referral?

Once a referral is submitted, our admissions team will:

  1. Review the information promptly
  2. Reach out to the individual (or referring party, as appropriate)
  3. Determine eligibility and level of care
  4. Coordinate next steps for assessment and admission

Heritage Treatment Link is a licensed 245G facility providing evidence-based treatment services for adults, including:

  • ASAM 2.1 – Intensive Outpatient Program (IOP)
  • ASAM 1.0 – Outpatient Program (OP)
  • In-Person and Telehealth options
  • Substance Use Disorder (SUD) treatment
  • Behavioral and Mental Health support
